Themanagementofincidentallydiscoveredkidneytumorshaschangedrecentlybecauseofimprovedunderstandingoftheirnaturalhistoryandlongercancer-relatedsurvival.Usuallyfoundasincidentalmassesondiagnosticimaging,smallkidneytumorsaredefinedaslesionsof4cmorsmaller(AmericanJointCommitteeonCancerstageT1a).Therehasbeena3-foldincreaseintheirdetec-tionduringthepastseveraldecadescoincidentwithincreasesinimagingutilization.Inthepast,theconventionaltreatmentformostkidneytumorswassurgicalresection,whichmitigatedrisksofcancerprogressionbutdidnotresultinanoverallsurvivalbenefitforsmalltumors.Morerecentevidencesuggeststhatnotalloftheselesionsshouldberesectedbecausesomearebenign,andforsomepatientswithsmallkidneycancers,thereisagreaterriskformortalityfromnononcologiccausesthanfromkidneycancer.
